To determine the appropriate services for each resident, we conduct a thorough in-person assessment. This process involves meeting not only with the resident, but also with their family members or legal guardian, as well as current service providers.
These collaborative conversations provide valuable insight into the resident and their specific needs. By engaging with all parties involved in the resident’s care, we ensure that we gain a clear understanding of their requirements and personal preferences.
This detailed assessment allows us to develop an individualized care plan that addresses the resident’s unique circumstances, ensuring that our team is able to deliver the most appropriate and effective services.

Early-Stage Dementia: Our team is experienced in assisting clients who are in the early stages of dementia, providing compassionate support and appropriate interventions.
Developmentally Disabled: We are equipped to serve individuals with developmental disabilities, focusing on fostering independence and well-being.
Mental Illness: Our care extends to those dealing with mental health challenges, ensuring a supportive and understanding environment.
In addition, we can assist clients in managing several chronic illnesses with support of their physician. Our approach emphasizes monitoring as ordered by their provider, medication management, and support for various health needs.

We provide a comprehensive suite of support services to ensure the comfort, safety, and well-being of our residents. Our offerings are designed to address both daily living needs and specialized care requirements, with a focus on maintaining independence and dignity. Services include:
- Medication Management - We assist with the administration and monitoring of medications as directed by the prescribing healthcare provider. We are also contracted with a licensed pharmacy which offers medication delivery directly to the home.
- Meal Preparation - Nutritious meals, based on a seasonal cycle and resident preferences, are prepared to accommodate individual dietary needs.
- Housekeeping - Regular cleaning and upkeep of living spaces help ensure a clean and comfortable environment.
- Laundry Services - Personal laundry and linen services are provided to maintain hygiene and comfort.
- Arranging Transportation - We coordinate transportation for medical appointments, errands, and community activities.
- Assistance with Managing Provider Appointments - Support is offered in scheduling and managing healthcare appointments to ensure continuity of care.
- Personal Care Assistance - Our team provides help with activities of daily living, including bathing and incontinence management, to support residents’ personal hygiene and well-being.
- Activities - A variety of activities are offered to promote engagement, socialization, and enrichment, including one to one sessions for residents who prefer this type of engagement.

Our administrative team and caregivers work with each resident, their family/guardian, and care professionals to develop a personalized care plan, that meets their unique physical, social and spiritual needs and preferences.
Developed during the admission process, these plans are reviewed and updated as needed to ensure the resident's needs are being met and preferences are clearly addressed to provide personalized care.

For residents who require additional or specialized care, we facilitate access to a range of services delivered by licensed professionals via a physician order in the comfort of home.
Services include:
- Therapies such as Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy (ST), and Wound Care
- Podiatry services
- Audiology and Ophthalmology services
- Hospice services

Our staff members are trained to ensure they provide the highest standard of care to our residents. Each staff member is required to appear on the Wisconsin registry as CBRF certified or to hold CNA certification accompanied by the appropriate CBRF training. This ensures that all staff possess the necessary credentials to provide resident care.
Ongoing development is required by our organization. All staff participate in training sessions to maintain and enhance their skills and knowledge regarding our residents. These sessions are designed to keep staff informed of best practices and updated care protocols, supporting our commitment to quality and safety.
Before joining our team and providing care in our homes, every staff member must complete a background check conducted by the Wisconsin Department of Justice.
Additionally, team members are required to complete an orientation process to ensure they understand their job requirements and the wants and needs of each resident they will be providing services for. This helps ensure the well-being of all of our residents.
